Booking a call

The booking calendar is provided through Xtract. Information you submit there is handled by Elevétion and its booking provider to process your request and manage appointments. Review the notices and consent options displayed in the booking form before submitting.

Information passed to the calendar

If you arrive using a link containing contact details, this website may pass your name, email address and phone number to the calendar to prefill the booking form. Campaign parameters may also be passed along to identify the source of your visit.

Health information

A booking request is not a medical assessment. Please use the care team’s designated intake process for medical records and detailed health information.
